Cirrhosis is characterized by regenerative nodules surrounded by dense fibrotic tissue. Symptoms may not develop for years and are often nonspecific (eg, anorexia, fatigue, weight loss). Late manifestations include portal hypertension, ascites, and, when decompensation occurs, liver failure. Diagnosis often requires liver biopsy. Cirrhosis is usually considered irreversible. Among the growth regulators are cytokines and hepatic growth factors (eg, epithelial growth factor, hepatocyte growth factor, transforming growth factor-alpha, tumor necrosis factor). Insulin, glucagon, and patterns of intrahepatic blood flow determine how and where nodules develop Angiogenesis produces new vessels within the fibrous sheath that surrounds nodules. These vessels connect the hepatic artery and portal vein to hepatic venules, restoring the intrahepatic circulatory pathways. Such interconnecting vessels provide relatively low-volume, high-pressure venous drainage that cannot accommodate as much blood volume as normal. As a result, portal vein pressure increases. Such distortions in blood flow contribute to portal hypertension, which increases because the regenerating nodules compress hepatic venules. The progression rate from fibrosis to cirrhosis and the morphology of cirrhosis vary from person to person. Presumably, the reason for such variation is the extent of exposure to the injurious stimulus and the individual’s response. So not all patients with end stage liver disease need Liver transplantation. So patients who have cirrhosis, but are not symptomatic or patients who have cirrhosis and are well compensated do not need a Liver transplant. Only patients who have got cirrhosis and who have got ascites, which is uncontrolled with the use of diuretics or patients who have jaundice or hepatic encephalopathy. That means the patient can start having drowsiness or they can be so severe that they can start going into coma. this is another form of decompensation or they can have bleeding through the upper GI or the lower GI system or they can have individual organs affected because of liver disease like the lungs can get affected or the heart can get affected because of end stage liver disease. That means the complications arising due to cirrhosis are actually the indications for liver transplant. So this is classified into Child’s A, B,C based on the symptoms and the blood test that we do in these individuals. Child A patient do not need a liver transplant unless they have a hepatocellular carcinoma. That means these patients who have end stage liver disease, have higher risk of developing cancer and patients who have liver cancer and cirrhosis in the background will need a Liver transplant. In the next group, that is Child’s B and C. Early Childs B can be managed medically but once they reach Child’s CTP 8 and above, then these are the patients that need Liver transplant.
